It is important that you complete all of the forms and bring them with you to your appointment. Please also bring photo ID and your insurance card(s). This includes Medicare and DSHS cards. Failure to present your card(s) will release Puget Sound Gastroenterology from any responsibility for incorrect or timely filing of contracted claims. It is the patient’s responsibility to notify our office immediately if there is any change to your insurance information.
If your insurance plan requires a written referral from your primary care practitioner, it must be in our office at the time of your appointment. This is your responsibility. If we do not have your referral we may need to reschedule your appointment. Please call your primary care practitioner’s office to make sure that this is done.
If your insurance plan has an office visit copayment, this amount will be required at the time of your visit. Please come prepared to make this payment. We accept cash, checks, Visa, and MasterCard.
Please bring a list of all medications you are currently taking. We will need to know about over-the-counter medications, as well as ones prescribed to you. Please ask your primary care practitioner to fax recent clinic notes, lab test results and any pertinent radiology test results to the office where you are scheduled prior to your appointment.
